A medical emergency can occur when there is a blockage of airway, stoppage of breathing or circulation, severe bleeding, severely broken bone, deformity, dislocation, any injury to the head, neck, or spine, loss of consciousness or seizures, heat stroke, severe asthma attack, or a severe allergic reaction.

In case of a medical emergency, follow the procedures listed below:

  • First responder is to complete primary survey and assess patient vitals. Call 911 first if alone and give relevant information. Then perform immediate first-aid/CPR as required. Continue until you are too exhausted to continue, are relieved by another qualified provider, or EMS arrives.
  • If possible, first responder is to direct another able person to activate EMS by calling 911, alert SHC personnel for assistance and return with AED.
  • EMS will require the following information: Nature of the injury/illness, location of the injured party, care that is being given and the address -5916 SW Nyberg Ln. Tualatin, OR 97062
  • SHC personnel are to alert a member of SHC Management or Person in Charge (PIC).

Management will take over assigning the duties listed below:

  • Retrieve the AED and bring it to the scene.
  • In the case of a youth emergency, alert the parent.
  • Dispatch a staff member to assist EMS
  • Staff member to flag EMS on Nyberg Ln to the West Parking Lot or most appropriate location based on the incident.
  • Staff member is to escort EMS through the building to the elevator if necessary & site of the incident
  • Direct available SHC staff members to clear the surrounding area of onlookers for the privacy of the injured party and ensure a clear path for EMS
  • Once EMS has taken over and no further assistance is needed, the first responder is to complete an incident/Accident report. Incident/Accident report is then emailed to Director of Business Services, COO & relevant Department Manager if necessary.
  • Ensure proper disposal of soiled materials/linens, items of clothing.
  • Contact facilities for any post-event cleanup needs

Bloodborne Pathogens

Bloodborne Pathogens (BBP) are defined as blood-borne diseases that can be spread through contamination by blood and other body fluids. The most common examples are HIV, hepatitis B, hepatitis C, and viral hemorrhagic fevers

It is important when cleaning & disposing of BBP to follow the following protocols to limit exposure:

Use the Bloodborne Pathogens Clean-Up Kits!

When handling BBP use the BBP Clean Up kits that can be found at either of the two Member Service Desk Locations:

  • Always use appropriate personal protective equipment (PPE) and handle, sanitize and disinfect BBPs assuming that they are contaminated
  • Disposable Gloves - will be worn when there is a potential for the hands to have contact with blood and/or body fluids.
  • Utility Gloves - will be used for cleaning purposes only, and may be disinfected for reuse if the integrity of the glove is not compromised.
  • Masks and eye protection will be worn when there is a potential for splashes, sprays, spatters, droplets, or aerosols of blood and/or body fluids.
  • Tongs for picking up needles and other sharps.
  • Used needles and other sharps will not be sheared, bent, broken, or re sheathed by hand. They will be placed in the Sharps Container.
  • Inform the Facilities Manager so the container can be replaced.

Thoroughly sanitize any exposed areas with disinfectant.

Dispose of all gloves, disposable PPE & contaminated cleaning materials in a red Hazardous Materials plastic bags. Hazardous Waste bags are to be placed inside the Hazardous Waste Receptacle located in the Pool Storage Room.

Following contact with blood and/or body fluids, employees will wash their hands with soap (antiseptic cleaner) and water immediately, or as soon as possible after removing gloves or other personal protective equipment.

Should you become exposed to BBP through the eyes, mouth or an open wound, contact your immediate supervisor on shift (Director, Manager, PIC) and follow the protocols listed in the BBP Program Binder kept in the Administration Office.

Evacuation Procedures

In the case of an evacuation, it is important to remember that SHC will never ask you to take heroic measures.

  • Evacuate your area calmly informing members in a loud voice that we are evacuating the building.
  • Assist members by alerting them to the situation and calmly usher them along with you.
  • Quickly find your nearest, clear & safe emergency exit and report to the muster area in Brown’s Ferry Park across the street from the club.
  • Once in the Muster Area both members and staff are not to leave until SHC Management or EMTs release you to do so.
  • Assist the Childcare Staff by helping to build a protective barrier around the children. Parents are allowed in but may not remove their kids until we have returned from the building or have been released by SHC Management or EMT personnel.

Lost Child Procedure

If a child goes missing while in the care of the club ie: childcare, camps, program begin a quick search of the immediate area including bathrooms, closets, under tables etc.

If a child is confirmed missing or reported missing, initial a Code ABC (lost child)

  • Obtain a detailed description of the child, including what he or she is wearing.
  • Inform SHC Management and/or Person In Charge of the lost child
  • Radio "Code ABC" Describe the child's physical features and clothing.
  • Facilities Team Members are to immediately report to the club’s East and West driveways to stop all cars from leaving the property
  • SHC Managers & Team members will immediately stop working, look for the child and monitor front entrances to ensure the child does not leave the premises
  • If the parent or guardian was not the individual who reported the missing child, find and inform them of the situation.
  • Call law enforcement if the child is not found within 10 minutes.
  • If the child is found and appears to have been lost and unharmed, reunite the child with the searching family member.
  • If the child is found accompanied by someone other than a parent or legal guardian, make reasonable efforts to delay their departure without putting the child, staff or visitors at risk. Immediately notify law enforcement and give details about the person accompanying the child.
  • Cancel the Code ABC via radio after the child is found or law enforcement arrives.
  • File an incident report according to the Incident Report SOP

It is important that the Incident Reports are completed professionally and factually with as much detailed information as possible that is pertinent to the incident. Be clear as to what medical care was given, if any, and keep personal opinions out of this document.

An Incident Report should always be completed at the time of incident or as soon as possible thereafter and reported immediately to SHC Management, Department Manager or PIC.

  • An injury resulting in medical care or the supplying of First Aid care or supplies to a member, guest or staff member.
  • An altercation between individuals or a group.
  • Any suspicious behavior by a member or guest that a Director or MOD deems appropriate to report.

For a member/guest incident:

  • A blank copy of the Member Incident Report can be found on the Shared Drive folder titled “Incident Reports”.
  • Complete the report fully and as factually as possible. Make sure to include detailed descriptions of the area surrounding the incident or equipment that was involved as well as any safety concerns or hazards.
  • Include detailed information regarding any medical care given or if EMS was offered or called.
  • Completed Incident Reports are to be taken to the Front Desk and immediately scanned and entered into the Incident Report Smartsheet.

For an Employee Incident or Injury

Blank Employee Incident Reports can be found in the Shared Drive folder titled "Incident Reports".

Employee Incident Reports are to be taken to the front desk to be scanned and entered in to the Incident Report Smartsheet.

If an employee injury is serious enough that care is needed from a medial provider, notify your manager or a director immediately as further documents will be required.
